El carismático Charlie Harris (Michael Shanks), jefe de cirugía del Hospital Hope-Zion de Toronto cae en un coma profundo, dejando al hospital sumido en el caos, y a su novia y colega, la cirujana Alex Reid (Erica Durance), en estado de shock. Junto al recién llegado cirujano, el Dr. Joel Goran (Daniel Gillies), y los doctores Shahir Hamza (Huse Madhavji), Gavin (Kristopher Turner) y Maggie (Julia Taylor Ross); Alex lucha por salvar la vida de Harris mientras éste, sumido en su coma, recorre el hospital sin saber si es un sueño, o es su espíritu o su propia imaginación.
